Providing Providers Resources to Manage Patients After Acute Care

Background:

The Gateway Medical Society and its member physicians are committed to the elimination of health disparities among minority populations. Our strategic goals include:
  • Provide continuing education programs for health professionals
  • Facilitate the communication, collaboration and networking among health professionals
  • Provide forums for the discussion of issues which aid in the advocacy for health reform and minority health issues
  • Increase minority representation in the healthcare workforce
Objectives:
  • Define types of facilities and resources to care for patient when the acute illness is stabilized, but continued care is still needed in the post-acute hospitalization
  • Review post-acute levels of care available, including rehabilitation, palliative services, home health and long term care
  • Understand post-acute care transition considerations for patients who are in special population categories including: behavioral health and/or substance use disorders and homelessness
Who Should Attend:

Provider Network audience including: physicians, nurses, social workers and other multidiscipline network and community based professionals.

  • Keep up to date on strategies to reduce health disparities
  • Increase the cultural awareness of your staff and colleagues
